Also, what is the argument that it’s nearly balanced? What happens if I introduce, in order, 0xfeeee…ee, 0xffeee…ee, 0xfffee…ee, 0xffffe…ee … 0xfffff…e, 0xfffff…ff? Wouldn’t that create a tree of depth ~256 with ~256 elements?

I think that this concern is right.
Correct me if I’m wrong, but following the algorithm steps I ended up with such tree.
